Kindly complete the form provided immediately after booking, ensuring you have your gift certificate code available if applicable. Should your gift certificate not cover the full cost of the session, any outstanding balance will be due at the time of checkout in-office. Please bring form or payment with you.

If a late cancellation or no-show happens, your gift certificate will be redeemed, and any outstanding amount will be charged to the card on file. In the event of a late cancellation requiring an additional charge, if the payment cannot be processed on the card on file, booking future services with Massage is Life will be restricted.
